RELIGIOUS AND SPIRITUAL HISTORY ASSESSMENT TOOL

Dr.Kehoe has created a tool to enable both medical professionals and mental health professionals to inquire about a person’s spiritual or religious history. Asking such questions as:

• Are you currently affiliated with a religious tradition? If so, which one?
• What is that like for you?
• Were you ever a part of a religious tradition? If the person responds positively then to ask what prompted the departure?
